Twinsburg Township, OH (January 13, 2025) – A Cleveland man lost his life in a workplace accident last Friday morning, January 10, while conducting an air quality assessment inside a railroad tanker car in Summit County.
The Summit County Sheriff’s Office identified the victim as 35-year-old Dalion Ambler, a contractor for Clean Harbors. Ambler was working at the Emerald Transformer facility when the incident occurred. According to reports, the call for assistance was made at approximately 9:45 a.m. after Ambler entered the tanker and became unresponsive.
Colleagues waiting outside the tanker made immediate efforts to help, but Ambler was ultimately pronounced dead at the scene at 12:45 p.m. Emergency teams, including Summit County HAZMAT and the Summit County Technical Rescue Operations Team (TROT), responded to the call. They ventilated the tanker and increased oxygen levels to a safe range before recovering Ambler’s body.
Investigations revealed that Ambler had entered an oxygen-deficient environment, which OSHA defines as containing less than 19.5% oxygen. The tanker, parked on a side rail behind the facility, was being tested and cleaned at the time of the incident. OSHA representatives are currently investigating the fatality. No other injuries were reported.
We extend our deepest condolences to the family, friends, and colleagues of Dalion Ambler during this difficult time.
Workplace Accidents in Ohio
Workplace accidents are an unfortunate reality for many industries, particularly those involving hazardous environments. In Ohio, OSHA investigates numerous fatal incidents each year, with confined spaces, such as tanker cars, being particularly dangerous due to risks like oxygen deficiency and toxic gases.
Here are some facts about confined space safety and workplace risks:
- Confined Space Hazards: These areas often lack adequate ventilation, leading to oxygen deficiency or the buildup of dangerous gases, which can be deadly without proper precautions.
- OSHA Safety Standards: Employers are required to adhere to strict safety protocols, including air quality testing, ventilation, and providing personal protective equipment (PPE).
- Emergency Preparedness: Proper training for workers and having emergency response plans in place can significantly reduce fatalities in hazardous environments.
- Contractor Risks: Workers hired for temporary or specialized tasks may not always be familiar with the specific hazards of a site, increasing their vulnerability.
Workplace safety is a shared responsibility between employers and employees. Ensuring compliance with OSHA guidelines and thorough training can help prevent future tragedies.
